There's clearly some way to do this but I can't work out how to have a link as a picture rather than text.

Just put the [/img] brackets in between the [/url brackets.

 

So insert the image, highlight it, click to insert a link and paste your link over the image link. I hope that explains it, not sure if it comes across clearly.
