I've always wondered with their (very rare) insta adds how these come about as I thought they actually surveyed/tested their new adds before they're playlisted so they make sure their target audience likes the song? They can't do that with songs added immediately after release so perhaps they have a %quota of risk taking allowed? E.g. it's an established artist that they already know is popular?
