It gives you poll title and poll content. Insert what you put in the topic title into the poll title, topic description goes into the poll content.

 

Next you get the option to add poll choice. Click that and you get a box to put the song title or whatever you wish to go in there. Keep adding poll choices until you finish (number of songs, options). Put a post in the box (if a survivor, the rules for example). Post it and it should work.
