Yes I worry about him too. I don't think he's ever been good at looking after himself.

Look again Elisabeth, he is wearing baggy clothes, I think he gets dressed in the dark in the wardrobe , his neck is skinny, it's all the shapeless baggy clothing that makes him look bigger . He should be dressed in a nice t-shirt & jeans & he would look fine.
