My thoughts are that it doesn't get enough love and I think, in time, this will "age" to be viewed as one of the strongest from the album. It kinda came and went in a few weeks before the Christmas influx and then Abracadabra is so undeniably huge / the album had many more talking points, that I think people often talk about Vanish Into You, Garden of Eden, Zombieboy etc before they mention this.

But, I love when Gaga is able to LET RIP vocally and the final chorus is a fine example of that and I still very much love how this set up the era as a whole.

‘Disease’ remains one of the strongest lead singles that she’s released, and I echo absolutely everything that Rob has said.

I don’t think the label quite anticipated how huge ‘Die With A Smile’ became (one of the biggest hits ever), and ‘Disease’ unfortunately became subjected to a cloud upon its release. That and then timing of it being released before Christmas, then eaten up again by the mammoth ‘Abracadabra’ with the eventual release of the ‘Mayhem’ album made it a distant memory.

Sonically, it is one of Gaga’s finest vocally albeit the lyrics aren’t exactly the most original but nevertheless her vocal delivery alongside the production makes the entire song what it is.

Still so incredibly good, I love that industrial sound and was hoping for more of it on the album (even though the album is still pretty much perfection for me anyway)

It's a shame it didn't perform a bit better, but I can kind of see why as it's a bit "bold" and "out there" for the wider general public to appreciate.
