Probably both. Gaga said in the radio interview about the video that because she has to walk so far her feet start bleeding and her shoes become full of grass and dirt.

So it is real blood.

I don't think it's real blood, since Gaga didn't actually walk from New York to Nebraska for the video. :lol:
